Pre-Application Questionnaire

The purpose of this questionnaire is to determine if you are a viable candidate for the position of Firefighter/EMT. If it is determined that you are a viable candidate, you will be automatically directed to the next step in the process.

Answer the following questions accurately, completely, and truthfully to be considered for employment:


Are you a United States Citizen?	YesNo
Will you be under the age of 18 years at the time of your test date?	YesNo
Have you ever been convicted of a felony, including pleas of nolo contendere?	YesNo
Have you ever been terminated for cause by any federal, state, county, or municipal civil service or merit system?	YesNo
Have you filed for more than one bankruptcy within a ten year period prior to the date of application, or do you have a bankruptcy in progress at the time of application?	YesNo
Do you have a less than honorable military discharge prior to the time of application? Select "No" if the discharge has been upgraded to honorable.	YesNo
Do you have any convictions or pleas of nolo contendere for Driving Under the Influence or Boating Under the Influence within five years of your application date?	YesNo
Do you have a driving history that shows a recurring pattern of traffic violations and may represent a disregard for traffic laws?	YesNo
Have you used marijuana within four years of your original application date or do you have any history of abuse?	YesNo
Have you been involved in selling, bartering, distributing, growing, manufacturing, storing, transporting, preparing or possessing any controlled drug which was in violation of Georgia law at the time of occurrence, whether the crime was detected or undetected?	YesNo
